For me, this is almost ALWAYS a result of a javascript error in the validation routine (typically a logic error like using = instead of ==) Firebug shows an error on line 58 with your else statement. Also, I'd pay particular attention to the input name line 47 ;-) (even though that's not the problem). I think the real problem are the semicolons at the end of your else clauses.
