ZDNet has a freebie application called Freedom of Association that
should suit.  It works quite well, though I mostly find it quicker to
use Winfile.exe or Windows explorer to hand modify a single file
association. This was in the readme:

It may be worth checking for a more recent version. This one works in
Win 98, though.
