Is this a static form that you want to create? IOW, is it the same for
everyone, they download it, print it, fill it out, and mail or fax it in? Or
is it something that needs to be customized for each visitor?

If it's the first - the static one - then you can create the form in a
program like Adobe Illustrator, or Page Maker, or Quark (or maybe even
Word), and most of them have the capability to save in PDF format. I don't
have all of the programs I just named, so I can't tell you their individual
functionality. Check under the "save" function of the program you are using
to create the form, and see if it has a "Acrobat PDF" option or format.
