Start:
- with client, define site goals & audience.

Next:
- with client, gather as much content or content info as you can.  & 
discuss navigation/link/functionality/maintenance issues.

Then:
- get out the paper & pencil, and proceed as others have described.

Clients hate this, by the way.  There is a tendency to think the 
designer should have some sort of omniscient powers & be able to 
understand the client's goals & needs when the client doesn't even 
understand them him/her/themselves.

IMO what slows things down is failing to do the preliminary planning. 
If you start out with the layout/gui, but lack an in-depth 
understanding of what is behind the interface in terms of 
content/audience/goals, you're a lot more likely to either end up 
backtracking at some point further down the road, or to simply end up 
with an inferior site.

It's hard to over-plan a web site in the earliest stages.
