All the below should be taken with a large amount of 'YMMV' Salt...
All of the PDF to HTML things, work great... unless, you have any kind of 
tables, footers, side notes or fancy formatting. Then they fall on their 
arses with a big splat!
The only reliable way is to get the original document (Word or whatever) 
and save-as, then clean the HTML, in whatever way floats your boat. But 
then again, if it's a really complex, Quark style document... you can 
probably forget making it into HTML unless you recreate it yourself
