i haven't checked the latest versions of
screenreaders, i hear they're getting better, but for
a long time there were screenreaders that just looked
at the screen and read from left to right (therefore
the 'don't use tables for layout' recommendations)
while others like pwwebspeak looked at the code.  they
could detect 'strong' and 'em' and read table cells in
the right order and also make sense of headers
(different thread).  

i guess you can give your headers, 'strong' and 'em'
any attributes you like in the stylesheet.  the
screenreader probably doesn't look at that, because
the style is not important to a blind person.  it only
looks at the html.  correct me if i'm wrong.
