You can do it with Quite some control in Photoshop two ways.

The first is to use the Mosaic filter under Pixelate. (This effect 
can be faded after the filter has been applied.)

The second is the Tiles filter under Stylize.  (This one gives some 
pretty great looking effects, just make sure to check the unaltered 
image box and start with low numbers in the boxes.)

Both these filters come with Photoshop.
