To that, I'd add:
*** Charge what the customer will pay ***

Seriously. Don't commoditize yourself/work.  Some companies will pay twice,
three times what others will pay.  The trick is in determining what that max
price is. (difficult)  Then, compare that price with the rate x hours =
price and if it is greater, do it.  (adjusting for other reasons such as
marketing, practice development, etc.)
