Well I've found that $0-$20/month hosts won't allow it at all;
$20-$100/month hosts have no problem giving you totally unrestricted
access; $100+ hosts won't allow it at all, unless you have a dedicated
server.

The reason is that mod_rewrite is dangerous - a badly written directive
can crash the server. Cheap hosts won't take the risk; medium hosts do
it all with VM servers so they don't care; expensive hosts won't take
the risk, unless you have a dedicated server.
