I've suggested they try a ticket system as a possible solution to this
problem. The requirements: free or really cheap, PHP or ColdFusion (or
Perl), MySQL or SQL Server, and, most importantly, it must be *dead
simple* to use :)

I am using 37Signals' Basecamp, and it has been going well.  Not free ($20 a
month for 10 projects), but well worth the price.  It's a hosted solution,
so there is no install and minimal setup.  The nice thing is that it's so
simple to use that clients have no trouble with it.  (37Signals does
useability consulting, after all).  They offer a free trial if you want to
check it out...
