I am just now in a very similar situation. I've found that the CSS Discuss
site [1] has a very good collection of different layouts. I'm working with
one of them now to adapt to my situation, which might be a bit different
from yours because I also want to maintain NN4 compatibility. But there are
plenty to choose from. I don't see any specifically with your requirements
for multi-sectioned header and footer, but from what I understand, that
should be easy enough to drop in if you've got the rest working. Check out
the "AlignLeftAndRight" topic [2].
