For small things I use rememberthemilk.com

For larger stuff I use basecamp,

and I have separated my directory structure to handle things accordingly.

\DATA\example.com\mysql\ -> the data files of example.com

\WEBS\example.com\ -> the development web root of example.com

\PROJECTS\example inc\example.com\inbox\ -> personal notes, ideas,
important things, screen designs, chat transcripts whatsoever of the
project that is mainly for my reference.

\PROJECTS\example inc\example.com\outbox\ -> just the opposite, things
that I send to the client, stuff that has more value to the client
than myself.

Though, I am quite new in this freelancing stuff (as a matter of fact
since I still have a full-time job I am not a true free (as in
freedom) lancer yet :) ),
I mean there may be other better practices. I don't know.
