[thelist] implementing a site survey using a lightbox / modal / popup

Hi all,

I need to add a short survey to a WordPress site using some kind of 
lightbox or modal popup window thingie.  I'd like to set a cookie so 
repeat visitors or those who have already completed the survey won't see 
it.  If you've successfully done this and are available / interested, 
please contact me offlist.
