enter your evolt.org username and password

if your username and password are accepted, re-enter your password and
confirm your new email address and url.  click go and the database will be
updated.  script will confirm your details and let you click to test the
redirect, which will mirror what http://members.evolt.org/<yourusername>
will do.

note that there`s no sanity checking whatsoever, and the email address bit
may yet be ditched, as theres currently no correlation between the email
system and the database storing this information.  this is just a demo
showing that the guts of the two scripts involved work well with the actual
info in the database.

it can be prettified and have sanity checking added quite quickly.
