i'm sure he does.  the voting/survey application that isaac and i are
working on will be able to handle these sorts of things (multi-page
questionnaires, anonymity (without allowing multiple votes), guaranteeing
(except when open to non-logged in users) single votes, "other" answers on
pick one/many type questions, etc.)

however, you might see if he'd be interested in hosting this questionnaire
being worked on right now if the timing doesn't fit with the completion of
the application isaac and i are working on.
