Ive read through pages and pages here for the past 3 hours, and cant get it working for some reason.

I just want to have a simple form, where the user can choose a file on their computer, press submit, and blamo, its e-mailed to me.

Can anyone provide me with a simple form and .pl to go along with it?  Ive tried a bunch allready, but nothing seems to work.
