I've been happy with using UML across the shop without any major issues. Which is funny - because at Uni I can remember me thinking to myself 'Damn, I will never use that, whats the point' ;o) Shows you how wrong you can be. As per requirements/spec - just a plain ol' Word Doc with an outline does the trick for me too. Depends on who I am giving it to - Business types, or Tech Types. Hope that helps (?)
