[thelist] HELP: "Form" for clients...

   First of all, sorry for my english. I dont know what
the technical term for what I am asking is :)

   I am moving fulltime to web dev/design and I want to create a form (a
paper one) that customers can fill with what they want in their site so we
can both agree on it. I dont know what this form is called, but I am looking
for help on designing mine.

   As you can tell, I am going for small sites. I made one site for a
friend, then a friend of my friend waw it and liked it, so he wanted me to
design his.. and a friend of his saw it and so on... so now I am in the need
of making the process more formal. 

   Any help is appreciated!
