This was on thelist not so long ago, so should be in the archives somewhere.

Basically, you need to put 4 files in the root directory of your CD.

AUTORUN.INF points to the .exe file
AUTOHTML.EXE looks for showhtml.ini
SHOWHTML.INI says to use index.htm
index.htm
