as in the user opens a PDF, fills out a series of form fields *within* 
the PDF, and then prints it?  or sends it?

or the user fills out a bunch of fields on a web page, and after 
hitting submit, it presented with a PDF with all the data from the 
form placed in the PDF in pre-defined areas?
