I'd like to point you to a form on our corporate site that may not be an end-all solution, but does help things out. http://www.instron.com/cgi-bin/suggestions.cfm All of the dropdowns (title, state/province, and country) include an option for "Other" at the bottom. If someone chooses "Other" they get a pop-up prompt window. I have extensive Javascript validation on this form. Also, if you use Dreamweaver, you can find lists of country/region/state/city codes for dropdowns at http://www.geocities.com/SiliconValley/Garage/2001/dreamweaver/ by clicking on the code section.
