I have 2 parts of the equation, maybe...

I have created explicit CSV output pages with PHP+MySQL, saved them, then
imported them into the Word for Mac ('98). I have added MySQL datasources
with the MySQL ODBC Driver to a WinNT box.

*Theoretically* you should be able to:
    - add an ODBC driver for MySql to your Windows box
    - use that as a datasource for your merge file
      (Windows help was pretty clear on how to use merged
       data, I'd read that part of the help section *carefully*)

I got the mysql odbc drivers from the mysql site --
