i agree that it occurs and is unfortunate.  that's why it's even more
important that we don't make the same mistakes.

agreed.  the situation with the 'next' button being the default is one i
hadn't considered.  in that instance you would want it to be the right-most
button (for obvious reasons) and still have it be the default.  (btw,
fantastic suggestion to use tables and rowspanning to get it in the order
you want.)
