What version of Visio do you have? I can send you a template with the title,
page number, section title, etc. all in there and then you can use that for
your project.
It's not that complex, if I understand you correctly.  Please email me

You need to use Background Templates, and then go into each page and set
File/Properties/Background and set the background to the one you need.  Even
your guides will carry over into the document targeted, so you can align
objects depending on the guides of the background.

Not sure I understand "each menu item"   - Send me a screenshot if you like.
