[thelist] Code QA Web Tools ?

I was curious if any of you could recommend a good web based code QA tool:

 - Web based
 - Multi-tier security - Used by coders, project manager, client
 - Simple - list views of code spcifications with related bugs, comments,
detail views
 - Allows basic reporting (not required)


I would prefer Coldfusion, ASP, but will gladly look at PHP or similar
solutions
