with dreamweaver it's due to one simple thing -- referencing a path to an
image in the rollover rather than an object that already has the preloaded
image attached to it.

from ie5.0 forward, cache settings will affect preloading.  for example, if
you have it set to check every visit to the page, then images won't be
preloaded -- ever.  changing this setting back to the default of
"automatically" and correct preloads will work once again.
