Generate the HTML or CSS that would do this usually.

Its what the HTML says that is important, not how you generate it
(PHP, by hand, Dreamweaver, etc). It seems likely that you have
introduced an error somewhere, in that situation I would take an
instant trip to http://validator.w3.org/
