[thelist] Telephone Number Format for the Many Different Countries

Does anyone know of or have good a good example of a web form displaying
telephone fields that would change based on the chosen country from a
dropdown list?  For example, if United States was selected then the
format would be (xxx) xxx-xxxxx.  However, if Ireland was selected then
the format would be ??????
 
Any help would be much appreciated.
