[thelist] Form generator/handler recommendation?

Does anyone have a recommendation for a utility to allow
the non-technical site owner to create a basic form (mostly "contact us"
style forms) with client-side validation, basic server-side validation and
then email/save to db the form data?

Oops, should have added that it would be best if it were in PHP or classic
ASP.
