There are a billion different ways to build a form.  You can use html, 
PHP, ASP and others.  You can process the form information by sending it 
to an email, a database, a log file or a combination of file processing.

In short there are lots of ways to set up a form and handle the 
information.  What determines it is the server the form is running on 
and the needs of the client.
