Sounds like dual license based on a CC Attribution Uncommercial Share
Alike license (http://creativecommons.org/licenses/by-nc-sa/2.0/)
and extending it with your particular terms of use.

Even if you find something that covers them all, chances are that you
may be in need to extend it a bit, or clarify what you understand (or
what you want the users to understand) from the terms.

So if I were you, I had better gone for a dual license.
YMMV of course.
