To answer my own question after googling (yet) again...

[?xml version="1.0" encoding="iso-8859-1"?]
[xsl:stylesheet version="1.0"
    [xsl:output method="html" indent="no" media-type="text/html" version="4.0"/]

    [xsl:template match="/"]
        [xsl:text disable-output-escaping="yes"]<!DOCTYPE HTML PUBLIC "-//W3C//DTD HTML 4.01//EN"

Use the "xsl:text" element. Sheesh.
