If your server isn't set up to parse php code then it will deliver the  php content as plain text, leaving the browser to make sense of it.

This would be the only case where a screenreader would have to deal with php code.

It would also be a very bad way to set up a webserver, as everyone can see your application code - the web dev equivalent of walking around with no pants on (not a good look)

PHP will output what ever you like - whether the html is accessible or not is up to the developer.
