Coming into this late...

Is this a Windows Apache? or Unix Apache?

You might be having issues with case-sensitivity. Turn on directory browsing for a bit and see what the server thinks it sees.

Also, do you have the proper allow overrides turned on?? If not, you will not be able to use the .htaccess file.
