What country are you located in?
isn't this a duplicate of 1.5 or does 1.5 mean something else?

"Other" text boxes could be in the table cell with the word 'Other' instead
of below.

While I personally love buttons labeled "Towel" and "Don't Panic!" I don't
think it's obvious enough. Except that there's only one button, so perhaps
it will be ass umed.

I'll be glad to re-proof or help code any way I can.
