The survey looks good. I took it.

Maybe if we state in the first paragraph how many pages it is, that
would be good. Doesn't it also have page numbers at the top. That would
allow people to see how much longer the survey is.

Here's some text if you want: "The survey is 8 pages long with about ten
questions on each page. You'll find section numbers at the top of each
page so that you can track your progress. The survey should, on average,
not take more than five minutes."
