Just quickly:

The button at top-left is pretty bad. The shape and textures don't fit with the
rest of the site at all. Personally, I really don't like the colour scheme, and
I'd avoid the red hover on lavender.

I'd also avoid centering titles, etc. And consider putting the printable
membership form on its own page and increase the spacing for people to write in
their details.

Finally, there are only so many rounded corners that I can take... consider
squaring opposing corners for a somewhat different effect. (ie, leave top right
and bottom left rounded, but square off top left and bottom right.)
