The radically less techy way of achieving this would be
to use the 'library items' feature of Dreamweaver and
insert the form as a library item onto each page.

Then, when/if you change the form the editor will
prompt you to approve the change in the other 49
pages.  No server trickery or SSIs involved.

I'm sure DW isn't the only editor which can do this...
