The last time I installed IIS4 .shtml support was there as std (along with 
.stm and .shtm)

Just checked my (nice clean) test server and yup, they're there.
BTW It's not the same as running an .asp file .asp, and some others, are 
mapped to asp.dll.
.shtml etc. are mapped to ssinc.dll
