You could just print the form to PDF, through Distiller, then use the form
tool in Acrobat to make you input areas. Make the same names as the HTML
form, and you won't have any problems. PDF forms can submit HTML or FDF
data. So if you have existing HTML forms, then you can submit HTML data from
PDF, with no one the wiser.

A new service exists that would handle the process, and make it simple, and
it talks to any database or format in existence, but may be more than you
need. If it is what you need just ask for more info.
