I was thinking about posting this same question to the list a while back.
This is the way Photoshop 6 acts right now when you set a guide in the
middle of a pixel.  When you go to snap your marquee against the guide
photoshop guesses which pixel (row or column) to snap it to.  If it's right
in the middle it can overshoot in both direction when cutting out two images
next to each other.  On a stack of images you can find yourself off by quite
a few pixels by the time you get to the bottom.  ImageReady doesn't do this
because it snaps all guides to the nearest pixel.

I hope Adobe addresses this very soon, but until then you have to zoom in on
your image (to the pixel level on the rulers) and reposition any guides that
are mid-pixel.  Or you can go into ImageReady and reposition your guides
(drag them away and then put them back) save the file, then cut it up there
or bring it back into PS if you want.
