You're right. 

Now thats odd... it's here in my version, but I cut it out of the email version. Must have been having one of those days (I can't actually remember what kind of day it was due to the brandy, but that's another story), or possibly getting a little over eager with Ctrl-K.

There are quite a few tools out there for generating PDFs, but one that deserves a mention is HTMLDoc (http://www.easysw.com/htmldoc/).
    
It can, build pdfs from multiple html files, using the h tags to generate table of contents automatically, be run via the commandline or in batch mode for on the fly generation of documentation, and it's really simple to get your head round.

Plus it's multi-platform and GPL (with a pay for support option).
