refreshing doesn't fix it for me.  the problem is that you're passing
indexes to the function and the index for the element refers to your hidden
input for the session id.  either change the index or pass the name of the
username field.

also, read carefully what james had to say on the topic.  he covered the
points of the error you're receiving very well.
