Hey Andrew,
Sure you can.
Create a checkbox in the form:
<input type="checkbox" name="addToList" value="yes">
Then on the receiving page...
if(isset($addToList) && $addToList == "yes"){
//assuming there's a form field called email
$throwItIn = mysql_query("insert into mylistofemails values('".$_POST['email']."');
}
