two (perhaps more) ways:

with apache, you can use mod_autoindex to do various index formatting
things:
http://httpd.apache.org/docs/mod/mod_autoindex.html

also, you can (supposedly, doesn't work for me) add headers and footers
with mod_autoindex as well:
http://www.evolt.org/article/A_Cheesy_htaccess_Tutorial/18/226/

or, with php(or whatever), there are lots of directory indexer scripts
out there.  i use one available here:
http://www.lophty.com/index.php?viewProjects=1&projstopicid=1 it uses
external css for formatting, so it's (sort of) easy to change.
