I'm seeing this in both Mozilla and IE, so I'm assuming it's
a web server issue.

Yah I've seen this vexing issue. Have you tried the usual? clearing out your
browser cache? refreshing (dumb question)? even possibly set content
expiration on your files? try renaming it to something else? deleting it on
the server side to force the issue? 

I'm sure others will chime in with other suggestions and I HTH,
