[thelist] Feedback Mechanism

I am looking for a way to integrate a feedback mechanism on the bottom
of all my subpages - something similar to the "Was this helpful" feature
you see at various websites.  I am not particularly interested in
implementing a custom-made solution and would rather outsource this type
of data collection and analysis.  

Does anyone have any suggestions of good companies that provide this
service?
