www.project.net claims to be able to offer alot of what you're looking for.
however, i've never used it, and i can't vouch for it's quality, or if the
actual Open Source app even really exists..

the app was supposed to have been released by now.

No single solution can address all your list.  I am afraid the only way is
that you select many packages, so single solution exists out there...

But, you could combine MS Project + MS Project Server + MS Exchange and you
would be close to Project Nirvana, but not cheap, and I know no one who has
used this whole set.

On the other hand if you want to compromise a little, why not BaseCampHP, it
does most of what you want, for an affordable price, but you need Internet
access to use it, check out:

http://www.basecamphq.com/

I have used it in the past in combination with MS Project (for the charts
and detailed planning) and it works great, but they are not interconnected,
you have to do that yourself.

For a simple project though like a website or medium web app, Basecamp is
more than enough for most people, highly recommended.
