I am currently running a survey for a non-profit using PHPSurveyor [1]. It has a quirky interface, but it is fairly comprehensive, and can be completely styled using templates, which means you could at least attempt to do a CSS based layout and get closer to making it accessible.

This is a true survey tool though, NOT an anonymous polling tool.

As others have mentioned, everyone seems to have good things to say about SurveyMonkey. I thought that it used tables for layout, which is one reason I hadn't looked at it much yet.
