I agree about Calibre - I use it as a conversion tool, but it's a bit flaky on epub ->pdf conversions. The first one took ages but once I'd got a template it was much easier to do. Because its pure xhtml/css you could edit it in a standard html text editor but some of the formatting will be ignored.
